Incident Narrative
An employee was in an aerial lift installing a section of angle iron for the installation of a new elevated walkway at a tank farm. There was a pipe racking system located behind the aerial lift's basket. When the employee retracted the lift basket it retracted into the racking system and the employee was caught between the lift's controls and the fixed racking system. The employee lost consciousness and sustained chest bruising.
