Incident Narrative
An employee was operating a press brake. The employee placed the part into the brake press opening to the backstop and actuated the ram lowering it onto the part. The employee's left hand then slipped forward off the edge of the part and up against the front of the punch where two fingers were pinched between the part being folded upward and the upper die surface. The fingertips were amputated and surgically reattached.
