Incident Narrative

An employee was pulling a piece of plastic off a bale of paper products at a loading dock. As the plastic broke free, he lost his balance and fell through the chain across the dock, landing on the ground about 2.5 to 3 feet below. He suffered multiple fractures to his shoulder and lower back.
