Incident Narrative
An employee was working on a utility pole from an aerial lift bucket. He fell from the bucket to the concrete sidewalk below, suffering a laceration to his head, a fractured shoulder, and broken ribs. Fall protection equipment was not worn at the time of the incident. In addition, the safety chains on the door to the aerial lift bucket were not engaged.
