Incident Narrative

An employee set up an extension ladder in the back of a building near a rooftop unit that needed service. While he was on the ladder a strong gust of wind from an approaching storm caused him and the ladder to fall approximately 10 to 15 feet onto the concrete sidewalk below. He fractured his right ankle and fibula, requiring hospitalization and surgery.
