Incident Narrative

An employee was standing on a 6- to 8-foot A-Frame ladder to change curtains in an ICU hospital room when he lost his balance and fell. He landed on a sitting chair that was next to his ladder, fracturing ribs on his upper right side and puncturing his lung.
