Incident Narrative

An employee went to close a breaking machine. When he grabbed the handle he reached over the top of the machine to secure the handle with his left hand. He then pulled the handle and lost his balance. His right hand slipped into the pinch point, which closed on and amputated his right pinky fingertip.
