Incident Narrative

While covering vent systems with plastic, an employee stepped onto a hole cover for a vent shaft and the cover gave way. The employee then fell from the 16th floor to the 5th floor in the vent shaft, fracturing both ankles and three vertebrae and suffering several lacerations.
