Incident Narrative

An employee was cleaning underneath a bulk tote dumpster using a compressed air wand when he noticed a piece of cardboard lodged underneath. He was attempting to blow out the cardboard when the machine cycled, pinning his arm between the piece of equipment and the floor. He sustained injuries to his left forearm that included a broken radius and ulna as well as lacerations to the skin. He was hospitalized.
