Incident Narrative

An employee and coworkers were finalizing repairs on a shot blast machine and had re-energized it. The injured employee was still troubleshooting the machine when someone pushed the start button, causing the machine to cycle. The employee's foot and lower leg were caught and crushed by the moving parts, requiring hospitalization.
