An employee was dumping shredded paper into a horizontal baler when the wire tie mechanism malfunctioned and did not tie a wire around a bale. He was attempting to fix the issue when his left hand was caught and crushed in the wire tie mechanism, requiring hospitalization. The baler was unguarded at the time of the incident.
