Incident Narrative
An employee was servicing a laundry press with the basket that surrounded the piston in the air to provide access. As the employee was tightening diaphragm bolts, the machine activated and crushed the employee's hands between the hydraulic piston press plate and laundry tub. The employee sustained amputation of the left hand, extensive fractures, and possible amputation of the right hand.
