Incident Narrative
An employee was installing a water jet holding fixture into a water jet when the fixture became slightly misaligned with the left locating pin. When the fixture was lined up and fell into place, the employee's glove and right middle finger were caught in a pinch point between the fixture base and the bed of the water jet, resulting in the amputation of the distal tuft of the right middle finger.
