Incident Narrative

An employee was entering a chicken meal silo to perform a 6-month cleanout when the silo sweep arm activated while the employee was inside. The employee's lifeline became tangled in the sweep arm, and the employee was pulled into the sweep arm and auger, suffering fractures and soft tissue injuries to the left scapula and humerus.
