Incident Narrative

An employee was doing routine minor maintenance on an automatic flagrod machine's gas meter when he noticed a flagrod was out of place in the machine's swage former. He was trying to push it into place when the head of the swage former came down on his hand, fracturing his hand, thumb, and middle finger.
