Incident Narrative
An employee boxed product in the fabrication packaging area and was picking up trash at the end of her shift. She was standing on a wet grate, so she put cardboard on top of it. The cardboard shifted, so she bent down to pick it up. At that point, her fingers were caught underneath the return line of a conveyor. Her left third and fourth fingertips were partially amputated.
