Incident Narrative

Two employees were pulling slate pieces (weighing 150 to 225 pounds) from a shipping crate that was stacked on another crate. The employees removed the side of the crate to remove the slate pieces. As they were removing a piece, it shifted and pinched the injured employee's left middle finger, amputating half an inch of the fingertip.
