Incident Narrative

An employee was fueling a truck and went into the store to get a receipt. He then entered the truck before exiting it again. The truck began to roll forward and he attempted stop it when his shoe was caught on something. The truck then ran over and broke his right ankle and fractured a bone near the little toe on his left foot.
