Incident Narrative

An employee was unloading tools and supplies from the bed of a pickup truck when the employee slipped on frost/ice that was in the bed of the truck. The employee fell off the bed and suffered a concussion, cervical and shoulder strains, and a tailbone fracture, requiring hospitalization.
