Incident Narrative

An employee was standing on a pallet being lifted by a forklift to change out a light fixture. He tried to cut into a live cable with wire cutters and was shocked. He then lost his balance and fell 20 feet onto metal pipes below, suffering a head fracture, shoulder fracture, and left hand burns.
