Incident Narrative

An employee was using an extension ladder to gain access to a section of a shredder to perform maintenance. The base of the ladder was inches away from the frame of a grapple crane. As he reached the top of the ladder and went to step onto the shredder platform, the ladder slipped back several inches into the crane frame and he lost his footing. He fell approximately 10 feet to the concrete below, striking his head and back.
