Incident Narrative

An employee was performing mechanical work on a roll-off truck by replacing the rear end on the axle. He was guiding the rear end on a chain hoist when the rear end slipped. His left hand was then caught between the chain and the rear end, lacerating his left ring and middle fingers. His left pinky finger was also amputated.
