Incident Narrative

An employee was standing on the ground near a pipe rack while other employees picked up a joint from a catwalk. The joint came off its cradle, rolling back onto the pipe rack and hitting the employee. He was pinned between two joints, suffering bleeding in his abdominal cavity.
