Incident Narrative
An employee was attempting to dump a tilt hopper. He held onto the area where the hopper slides onto the forks and attempted to push the tilt hopper to empty it. When it dumped, the hopper base pinched his finger between the fork and base, amputating the first digit of his left-hand pointer finger.
