Incident Narrative
An employee loaded a ladder into the back of a truck. Other equipment was preventing the ladder from laying flat in the bed of the truck. He entered the bed of the truck, rearranged the items, and positioned the ladder to set down in the truck bed. When he went to exit the truck bed, he placed one foot between the ladder and the inside of the truck bed wall, while the other foot was on the bumper of the truck. The tailgate was closed at the time. When he attempted to take his foot out of the bed, his foot became caught, causing him to fall from the bumper and truck bed. He broke his right femur.
