Incident Narrative

A crew was preparing to load a spool onto a trailer to spool up flex-pipe. An employee was trying to guide the center spool shaft into the trailer guide so he could place the guide-pin into the spool shaft when the spool shifted, trapping his left middle finger between the center spool and the guide place. His fingertip was partially amputated.
