Incident Narrative

An employee was unloading a trailer using a pallet jack. When the employee exited the trailer, the dock plate initiated an automatic reset (a feature of the plate by which it senses a potential trailer pull and will raise itself up to settle back into place in order to maintain a flush surface with the surrounding dock). When the plate began its reset cycle, the employee had already started to back up and while the plate was elevated, the employee's left leg contacted the plate edge fracturing the lower leg.
