Incident Narrative

An employee used a collapsible ladder to access a roof. After setting up the ladder and checking it for stability, he began to climb up the ladder. He reached the roofline and began transitioning to the roof when the ladder began sliding underneath his feet. He began sliding with the ladder, and his legs went between the ladder's rungs. He then fell backwards, landing on his back on top of the ladder and fracturing his T-12 vertebra.
