Incident Narrative
An employee was climbing up an extension ladder when the ladder's locking mechanism became disengaged and one of the ladder sections slid down and caught his left foot between the rungs. As the employee tried to remove his left foot from the rungs, the ladder began to fall and his left leg became caught between the rungs. He fell approximately 8 to 10 feet with the ladder to the gravel ground below, breaking his left leg.
