Incident Narrative

The injured employee moved a super sack of product to the crystallizer area by forklift. The sack is lifted by a hoist over the hopper of the slurry machine and the bottom of the super sack is cut. The employee was attempting to retrieve a piece of sack that fell into the operating hopper when his left hand became caught in the hopper, injuring his middle and ring fingers.
