Incident Narrative

An employee was working on a sample table, loading weights into the benefits inserts of a punch machine. She missed a weight in one of the inserts and shut the machine off to add the weight. The machine restarted and caught the employee's finger between the punch and the insert. Her left index finger was amputated.
