Incident Narrative
The injured employee was getting a steel plate positioned in the plate rack to place a plate clamp onto the material to remove it from the rack. Another employee was assisting by separating the plates to create room for the plate clamp. The pry bar slipped out of position allowing the plates to start falling back into their original position. As the plates were falling, the injured employee reached out to stop them and pinched the left middle finger between the plates resulting in a fingertip amputation.
