Incident Narrative

An employee was disassembling a ski lift drive terminal and went onto the terminal's catwalk to remove the nuts. When he removed the nuts, the plates swiveled and collapsed underneath him. He fell 12 feet, 9 inches, suffering broken ribs and a collapsed lung.
