Incident Narrative

An employee was guiding a forklift to set a roll of conveyor belting onto a stand on a table. The stand collapsed when the roll was set, and the employee was catapulted up and off of the table, landing 12 feet away on a concrete floor in a sitting position. He injured his right hip and buttocks and was hospitalized, and later had surgery to correct hip fractures.
