An employee was operating a press brake to bend a piece of steel. After bending the piece of steel, he re-actuated the press brake with the foot pedal while grabbing the part out of the die. The part broke up toward the front/top of the press (ram) and a previously bent flange caught and amputated his left index and ring fingertips above the knuckle.
