Incident Narrative

The employee was operating a shear to fabricate a part for a crane. The employee placed an aluminum sheet into the machine opening and accidently hit the foot control which activated the press. The employee's left middle finger was amputated above the first knuckle. The employee was treated at a local hospital and released the same day.
