Incident Narrative

An employee was re-painting a patio ceiling while standing on an extension ladder when the ladder became unstable and fell backwards, causing the employee to fall 15 to 16 feet to the ground. During the fall, the employee's face struck the ladder. The employee suffered a broken nose and cuts and bruises to the arms, legs, and hands.
