Incident Narrative
An employee was installing house wrap tape while working from a 300-pound-rated, 28- to 32-foot fiberglass extension ladder, which had filler lumber material elevating and supporting one side of the base ladder side rails to level the ladder base. During the house wrap taping operation, the ladder and employee fell an estimated 20 feet. The employee suffered wrist, elbow, arm, and rib fractures, as well as lacerations to the kidney and face.
