Incident Narrative
An employee was operating a blender when cheese became caught in the blender's discharge door. He was attempting to unjam the cheese blockage when a coworker noticed the blender was in the fault state and reset the fault, causing the blender to run while the employee's hand was in the discharge door. The blender crushed the employee's right hand, breaking and lacerating it. The blender was not locked/tagged out at the time of the incident.
