Incident Narrative
An employee was walking around a machine and stepping up and down to various positions to thread sheet film through the machine. He held the edge of the sheet film with one hand and stepped down to the floor from a 20-inch-high platform, resulting in tension on the film. While one of his feet was on the platform and the other foot was approaching the floor, the film in his right hand tore from the sheet. He lost his balance and fell to the floor. As he tried to break his fall, he broke his right arm and dislocated his right wrist when he hit the floor.
