Incident Narrative

An employee was washing a tank trailer while standing on a ladder mounted to the trailer when he fell approximately 10 feet to the concrete floor. He suffered brain contusions and hematomas, a skull fracture, vertebrae fractures, and a right scapula fracture, requiring hospitalization.
