Incident Narrative

The injured employee was cleaning in the mill room and was standing on top of a guardrail on a platform. A forklift operated by another employee struck the platform and the injured employee fell 6.5 feet from the guardrail he was standing on to the mill room floor below. The employee fractured both heels and the left ankle.
