Incident Narrative
An employee was dropping a fill cube into a basin when it was caught on bolts installed inside the basin. He pushed on the fill cube with his hand to free the fill cube. When the cube was freed it pinched his right hand between the casing panel and the basin, amputating his right index fingertip.
