Incident Narrative

On April 10, 2017, an employee was working on a door jam in an aerial lift beneath another aerial lift where a coworker was welding an angle iron. The angle iron tack welds were broken, causing the angle iron to swing downward and strike the employee in the upper back area. The employee suffered upper back and neck injuries and fractured vertebrae.
