Incident Narrative

An employee was torch cutting anchor bolts from the anchor plate of a stand-alone jib crane. As the employee cut the last of the anchor bolts, the approximately 10-foot-high round column with a 10-foot-long jib fell over onto a second employee standing nearby. The second employee was hospitalized with a fractured lower leg and lacerations to the back, neck, forehead, and leg.
