Incident Narrative
On March 16, 2017, an employee was changing evaporator coils on the roof-mounted cooling unit for the store's walk-in cooler. The employee was going up an extension ladder to access the roof from the street when the ladder slipped from under him and the employee fell approximately 12 feet to the ground below. The employee suffered a broken right leg above the ankle and a shattered heel bone. The employee was hospitalized.
