Incident Narrative

An employee was setting up the wire draw machine. The employee was stringing up the machine and using a pulling chain to thread wire through the dies. The employee ran the machine in full run mode and pulled the chain wire off. The employee opened the soap access door to look inside and was struck in the mouth and hand by the chain. The employee was hospitalized for a left middle finger laceration and a busted upper lip.
