Incident Narrative

An employee was attempting to replace the permeate port on the end-cap of a reverse osmosis vessel. The equipment retained pressure, which caused the end-cap to be forcibly removed. The pressurized end cap struck the employee's right hand against a support member, amputating two fingers.
