Incident Narrative
Two employees were operating a welder machine. The first employee jogged the welder to thread coiled metal into the machine. As the coiled metal gets threaded into the machine, protective paper comes off the metal. The second employee was picking up the paper when he lost his balance. His hand then contacted the coil and was crushed between it and the hold-down roller. His right index finger had to he surgically amputated. The machine was unguarded at the time of the incident.
