Incident Narrative

An employee was operating a machine to load wood slabs for processing into doors. As the employee was attempting to remove cardboard from the bottom of the slab being handled, the machine clamped onto the slab. The employee's left middle and ring fingertips were crushed and amputated between the slab and the clamp. The employee was hospitalized.
